The medical term for chronic care services provided to individuals with multiple chronic diseases is Chronic Care Management (CCM). Care coordination for patients with at least two chronic illnesses that are anticipated to persist for more than a year or until the patient’s death is known as CCM, and it is carried out remotely or outside of the clinic. For at least 20 minutes per month, non-face-to-face care coordinating services can be billed to CCM by physicians, nurse practitioners, physician assistants, FQHCs, RHCs, and certified nurse midwives. | |
